Jan Słowikowski (* 19. September 1915 in Nowy Sącz; † 13. December 2010 in Wrocław) was a polish physician, surgeon, urologist, oncologist. He was 95 years old. He studied at Jagiellonian University. He worked at Wrocław Medical University. He received 5 awards, including Commander with Star of the Order of Polonia Restituta, Gold Cross of Merit. He was buried at Holy Family Cemetery.
